Blue Screen of Death (also known as a blue screen or BSoD) is an error screen displayed on a Windows computer system after a fatal system error, also known as a system crash: when the operating system reaches a condition where it can no longer operate safely. When it occurs, the computer screen goes blue and then restarts.
Currently this problem is being seen with some of the practitioners using the USB or hardwired systems (not the portable Bluetooth systems). It appears to be specific to the version of FTDI driver software on their computer. The offending versions are 2.12.24 and 2.12.26.
SOLUTION: This problem can be resolved with a forced update to version 22.214.171.124 (or later).
